Early morning Appointments: Dr Rachael Moulden provides early morning appointments from 7.30am on Monday. Dr Praveen Jayaram provides early morning appointments from 7.30am on Thursday and Friday.
Appointments: Appointments can be made by phoning 07 3177 1040. Online booking services are available via HealthEngine. Please notify reception staff if an urgent appointment is required. It is a policy of our practice to ensure that acutely ill patients are seen promptly. Urgent cases are triaged and will be given priority. Our reception staff will attempt to contact you if there is any unforeseen lengthy delay. If more than one person from your family wishes to see the doctor at the same time, please make a separate appointment for each family member. If you require additional time please book a long appointment. This will assist in reducing waiting times for other patients. Consultations are by appointment, Monday to Friday and Saturday mornings. Every effort will be made to accommodate your preferred time and doctor. All appointments on Saturday morning are privately billed. Instant Medicare rebates are still available at the time of the appointment. Health Care Card and Senior Health Care Card holders are billed at a reduced rates except for Saturday’s and evening appointments
Payment: Payment is expected on the day, we accept cash, credit/debit Eftpos cards. We do not accept personal cheques
Our Online Easy-Claim system linked with Medicare enables instant Medicare rebate via debit Eftpos card, OR within 48 hours if your bank account details are held by Medicare. ** Please note that this instant refund is only available at time of consultation. Accounts not paid for on day attract an administration fee of $20.00 and must be paid within 5 days of issue of account. An administration fee applies for account reminders, postage, reprinting of referrals, pathology results or faxing same to specialist where patient has lost or forgotten to take with them to their appointment.
Missed Appointment / Cancellation Fee: Please notify the surgery if you are unable to attend an appointment well in advance (minimum 2 hrs notice) or the missed/cancellation appointment fee may apply.
Interpreting Service: If you or your family members require the services of an interpreter due to non-English speaking / hearing impaired, we can organize this for you. Please advise us when making your appointment.
Consumable Fee / Facility Fee: Patients are required to pay for consumables used for surgical procedures, dressings, ear syringing (miscellaneous/additional items). Facility fees cover costs associated with the use and maintenance of treatment room equipment. These fees are privately billed and non-refundable from Medicare.
Bulk-Billing Exclusions: Workcover Compensation, Travel Vaccinations, Medicals, Home Visits, Excisions Travellers / temporary residents / Brisbane visitors will be privately billed (including OSHC card holders) If you do not have a current Concession card in your possession or on our surgery records you will be charged full fees.
Aviation Medicals: Dr Cotter and Dr Campbell are Designated Aviation Medical Examiners
Staff Policy: We value our staff. As employers we are obligated to provide a safe workplace for our staff. Patients who verbally abuse or threaten our staff will be asked to leave the practice and seek their care elsewhere.
Test Results: Test results are reviewed by your doctor. For confidentiality reasons receptionists cannot give any results to patients at the front desk or over the phone. Please make an appointment after you have had your tests to return and discuss your results with your doctor.
IT IS THE PATIENT’S RESPONSIBILITY TO COME IN TO FOLLOW-UP THEIR RESULTS. YOU MUST RING / RETURN TO PROVIDE A FAIL SAFE WAY TO ENSURE YOUR RESULTS ARE SEEN AND ACTED UPON BY A DOCTOR Medical Certificates:
These will not be issued for non-medical reasons apart from bereavement or family support. The patient must be seen at time of illness to write a certificate. They cannot be backdated as it is illegal.
NO Schedule 8 scripts will be given on first visit without prior confirmation from previous General Practitioner. Current patients are required to be reviewed for continuity of care. Repeat Prescriptions (current patients): As of 1st May 2016 all repeat prescriptions will require a consultation with the GP. Modern medications are potent and can lead to harmful side effects if taken inappropriately. Regular reviews are required while on medication to check for any possible side effects, to receive updates on whether it is still the best one for your condition, and to remind you about interactions or new side effect warnings with any other medication you may be taking.
Work Cover: Work Cover related visits are privately billed at Work Cover QLD rates. You can claim reimbursement from your employer or directly from Work Cover QLD.
Referrals: (current patients) Medicare requires the GP to see and assess their patients before referring any patient to a specialist. Provision of a referral requires a visit to the doctor to ensure all information is current and relevant. By law, referrals CANNOT be backdated. Provision of an ongoing referral without a consultation is at the doctor’s discretion and will incur a $15.00 fee. There is no additional charge if provided in a consultation.
Telephoning your Doctor: Doctors may be contacted during normal surgery hours. A message will be taken and the reception staff will advise you regarding a return call. Although most problems are best dealt with in a consultation, a doctor will always be available during normal surgery hours in an emergency.
Recall System: This Practice encourages taking personal responsibility for your health. It is your responsibility to obtain your results. We are a very busy practice and cannot call all patients about their results. Non-urgent abnormal results will be discussed on your next visit. With results of a serious nature, if you fail to ring, return our calls, messages left, a recall letter will be sent but you will be charged an administration fee to cover costs.
Reminder System: Our Practice is committed to preventative care. Nundah Doctors Surgery utilizes a recall system. If you do not wish to participate in this system, please let your doctor know.
Out of Surgery Visits: Home visits are available in the immediate area for frail / aged patients of this Practice at the doctor’s discretion and availability. Fees may apply
After Hours Service: This Practice is registered with the Home Doctor Service in providing after-hours emergency service to our patients. Please ring 13 74 25 (13sick) to arrange for an after hour’s doctors visit when the surgery is closed.
Transfer of Records: We are happy to transfer your care over to your new doctor. There is a minimum fee per file of $30 or greater, depending on file size. Family files are transferred at a discounted fee. This fee covers the cost of photocopying, handling and registered post.
Parking: Parking is available onsite at the rear of our surgery or street parking available on Bilsen Rd. Wheel chair access available.
Management of your Personal Health Information: Your medical record is a confidential document. It is the policy of Nundah Doctors Surgery to maintain security of personal health information at all times and to ensure that this information is only available to authorized members of staff. At times your health information may need to be shared with other health care providers to provide you with the highest level of medical care. These may include Allied Health / Specialist referrals. Your consent will be obtained prior to sharing this information. We abide by the 10 National Privacy Principles available at: www.privacy.gov.au
